Job Title: Care Coordinator
Salary: £26,000 - £28,000 per annum
Location: Stoke on Trent
Working Hours: Monday to Friday, 9:00 AM to 5:00 PM (plus shared on-call rota)
About the Role:
Claire at Amber Mace is recruiting for an experienced and dedicated Care Coordinator to join a compassionate and supportive team. This is an excellent opportunity for an individual with previous experience in a similar role within the care industry, looking to develop their career while ensuring the delivery of high-quality care to the service users.
As a Care Coordinator, you will play a vital role in ensuring the smooth and efficient operation of care services for the clients. You will be responsible for coordinating care schedules, liaising with service users, families, and healthcare professionals, and managing a team of care staff to ensure that all clients receive the best possible care.
Key Responsibilities:
- Manage and coordinate the daily schedules of care staff to ensure appropriate coverage and service delivery. Prioritise urgent care needs and handle any last-minute changes in a timely and effective manner.
- Maintain regular contact with clients and their families, ensuring care plans are followed, and any concerns or requests are addressed promptly. Provide updates on service delivery and care changes.
- Support in the recruitment, training, and development of care staff. Ensure staff are fully trained and competent to meet the needs of clients. Provide ongoing support, supervision, and performance reviews.
- Ensure that individual care plans are created, updated, and maintained in line with client needs. Ensure all care documentation is accurate and up-to-date.
- Monitor and evaluate care delivery to ensure that it meets both company standards and regulatory requirements. Address any areas for improvement and make necessary changes to enhance service quality.
- Participate in the shared on-call rota, providing support to care staff and service users outside of regular working hours as required.
Key Skills & Qualifications:
- NVQ Level 3 in Health and Social Care – preferred
- Proven experience in a care coordination role, ideally within a similar setting
- Strong organisational and time-management skills, with the ability to manage multiple tasks and priorities.
- Excellent communication skills, both written and verbal, with the ability to interact effectively with clients, families, and staff.
- Knowledge of care planning, risk assessments, and relevant regulatory standards
- Ability to manage and resolve problems swiftly, particularly in high-pressure situations.
- A strong commitment to providing person-centred care and supporting the needs of vulnerable individuals.
- Computer literate with knowledge of care management software
Desirable:
- Previous experience working with care software and scheduling systems.
- Full, clean driving license
