Opportunity for a Qualified Physiotherapist to work within a Primary Care Community Team, Based in Stoke
The role is based within the Primary Care Team with the role comprehensive holistic assessment with appropriate onward referral as required. The applicant will be responsible to updating the patient record of each case where advice and guidance will be provided with clear personalised signposting to third sector organisations and self-help advice.
