Domiciliary Care Coordinator

Location: Sudbury, Suffolk

Salary: 28K - 30K per Annum + Bonus

Hours: Full Time - 40 hours / week - Days

Our client is on the lookout for a passionate and proactive Care Coordinator for this new Domiciliary Home Care Branch ready to lead from the front, in delivering exceptional, person-centred care.

Role Overview

As Care Coordinator - you will be supporting carers, coordinating care and working hand-in-hand with the Branch Manager and integrated care services, to ensure smooth, compassionate service delivery. Primarily community-based, the position will include office-based work as the branch expands.

Key Responsibilities

  • Deliver hands-on care in the community
  • Create & implement individualised care plans
  • Review & update care plans
  • Oversee staff rotas & medication management
  • Support training, shadowing & ongoing development
  • Share on-call responsibilities
  • Efficiently manage & distribute staff rotas
  • Act as first point of contact for clients and carers

Requirements

  • 2 + year's experience in care
  • Full UK Driving Licence
  • Empathy, organisation & leadership skills
  • Willingness to train and grow - undertake NVQ or further training as required
