We are seeking a dedicated and empathetic individual to join our Adult Social Care Service as a Support Broker within the Brokerage Team.

Support Brokers are instrumental in arranging a wide range of care services for individuals either in their own homes or within residential care settings. Working closely with Social Workers, you will use care and support plans to broker personalised care packages that align with the wishes and needs of the individual.

This role requires close collaboration with a number of internal teams, including Social Locality Work Teams, the Commissioning Team, START Team, Hospital Discharge Team, and Safeguarding, ensuring a seamless transition for individuals entering care. You will also engage with external organisations including voluntary sector partners, care providers, service users, and their families.

Key Responsibilities:

  • Arrange and negotiate care services in line with support plans developed by Social Workers

  • Maintain accurate and up-to-date financial records, ensuring value for money in line with Council policies

  • Operate the duty system, responding to telephone and email enquiries and referrals via the Mosaic system

  • Identify potential barriers to accessing appropriate services and facilitate access accordingly

  • Respond promptly and positively to customer enquiries, ensuring care packages are person-centred and outcome-focused

  • Refer safeguarding or quality concerns to the appropriate teams or agencies

  • Maintain knowledge of local services to provide informed advice to service users

  • Record all actions to meet agreed performance targets and standards

Essential Knowledge and Experience:

  • A thorough understanding of the Care Act 2014, Mental Capacity Act 2005, and the Deprivation of Liberty Safeguards (DoLS)

  • Strong knowledge of safeguarding legislation and associated policies

  • Significant experience within the care sector

  • Proven ability to work with empathy and a strong customer-focused approach

  • Skilled in building positive relationships and influencing outcomes through negotiation

  • Ability to maintain accurate records and use IT systems proficiently
