About the Role
We are seeking a dedicated and skilled Locum Clinical Pharmacist to join the Sutton Primary Care Networks (PCNs). This role is based in a GP Practice in Sutton Cheam, where the post holder will be responsible for supporting the PCNs in delivering expert medicine management, optimisation, vaccination, and supporting patients with their medications. As part of a multi-disciplinary team, you will work closely with GPs, nurses, and other healthcare professionals to enhance patient care.

You will also be enrolled in the 18-month Primary Care Pharmacy Education Pathway with the Centre for Pharmacy Postgraduate Education (CPPE) and will have opportunities for further development, including non-medical prescribing qualifications.

Key Responsibilities

  • Clinical Responsibilities:
    • Conduct long-term condition clinics (e.g., Respiratory, Cardiovascular, Diabetes).
    • Provide structured medication reviews, including polypharmacy and managing chronic conditions.
    • Manage medication after hospital discharge and perform reconciliations.
    • Identify high-risk patients and manage their medication in collaboration with the healthcare team.
    • Ensure continuity of medicines supply to vulnerable groups.
    • Lead clinical audits, quality improvements, and training initiatives.
    • Support public health campaigns and ensure compliance with CQC standards.
  • Clinical Medication Review:
    • Undertake medication reviews with patients (patient-facing or over the phone).
    • Reduce polypharmacy and wasteful prescribing.
    • Implement national and local guidelines to improve the quality, safety, and cost-effectiveness of prescribing.

About You
To be considered for this role, you will have:

  • Minimum 2 years post-qualification experience in a hospital, community, or general practice setting.
  • Experience in conducting medication reviews and offering patient counseling.
  • A strong understanding of long-term conditions and experience in their management.
  • Ability to work collaboratively within a multi-disciplinary team.
  • Registration with the General Pharmaceutical Council (GPhC) and a postgraduate qualification or evidence of continuing professional development.

Desirable Experience:

  • Experience working with NHS IT systems, including Emis.
  • Independent Pharmacist Prescriber accreditation.
