As a Social Worker in the Transition Team, you will work with young people transitioning from children's services to adult services, assessing their needs and providing support during this critical phase. Your role will involve conducting assessments under the Care Act 2014, developing personalised care plans, and facilitating access to services that promote independence, well-being, and long-term support. You will collaborate closely with the young person, their families, and multidisciplinary teams to ensure a smooth transition into adult services, while adhering to statutory requirements and safeguarding procedures.

Responsibilities include:

  • Monitoring, reviewing and maintaining up to date support plans, ensuring they meet the changing needs of young people and reflect good value for money whilst advocating for young people's rights, choices, and access to services
  • ensure that children who are transitioning to Adult Services have the right support from the right people as they prepare for adulthood
  • Examine and supervise carer strategies.
  • support with assessment and support planning for high cost placements and packages of care for young people who are approaching adulthood
  • Safeguarding and promoting the welfare of young people, adhering to relevant legislation and policies

Requirements:

  • 2 years experience post qualification
  • Action safeguarding procedures
