As a Qualified Social Worker in our Locality Team, you will have the opportunity to work directly with individuals and families within the community, providing essential support and intervention. You will play a crucial role in promoting their well-being, independence, and ensuring their needs are met.
Responsibilities:
- Undertaking assessments of individuals and families to identify their support needs and develop care plans.
- Providing direct support, counseling, and advocacy to individuals, ensuring their voices are heard and their rights protected.
- Collaborating with partner agencies, community resources, and healthcare professionals to coordinate and deliver holistic services.
- Safeguarding vulnerable individuals, adhering to relevant legislation and local policies.
- Regularly reviewing and monitoring support plans, adapting them to meet changing circumstances.
- Maintaining accurate and up-to-date records, ensuring confidentiality and data protection.
- Participating in team meetings, case conferences, and multidisciplinary forums to share information and contribute to collaborative decision-making.
- Keeping abreast of relevant legislation, policies, and best practices in social work.
Requirements:
- Qualified Social Worker with current registration with Social Work England.
- Experience working in a locality or community-based social work setting, supporting individuals and families.
- Sound knowledge of relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005.
