What you will do:
- Conduct comprehensive social care assessments in adherence to regulatory guidelines and best practices.
- Develop, implement, and monitor personalised support plans in collaboration with individuals and their families.
- Undertake regular reviews of care plans to ensure that services remain appropriate and meet changing needs.
- Lead safeguarding initiatives by conducting risk assessments and facilitating protection planning meetings.
- Carry out mental capacity and best interests assessments, along with Court of Protection applications.
- Conduct carers assessments and reviews, ensuring that the needs of carers are identified and supported.
- Facilitate NHS Checklist and Decision Support Tool meetings to determine eligibility for continuing healthcare.
