Sutton Council is seeking a dedicated and experienced Qualified Social Worker to join their Adult Services Transition Team. This is a fantastic opportunity to make a real impact on the lives of young people with additional needs as they move into adulthood. About the Role: As a Qualified Social Worker, you will work closely with Children's Services, Education, and Health to ensure young people receive the right support at the right time. Your primary focus will be on assessing and supporting high-cost placements and care packages for young people preparing for adulthood. Key Responsibilities: Conduct Care Act 2014 compliant eligibility assessments for young people and their carers. Develop person-centred, strengths-based support plans that ensure the best outcomes. Carry out Continuing Healthcare (CHC) checklists and collaborate with NHS partners for funding where eligible. Monitor, review, and update support plans to reflect changing needs and provide value for money. Advocate for young people's rights, choices, and access to services. Undertake safeguarding investigations and support individuals at risk, including Court of Protection work. Manage a complex and varied caseload, making sound professional judgments with appropriate supervision. Work collaboratively with multidisciplinary teams across children's services, healthcare, education, and community partners. Why Join Us? ?
