- Details: 12-week booking with potential for extension.
- Hours: Full-time, Monday to Friday, 8:30 AM - 4:30 PM
Key Responsibilities:
- Provide physiotherapy services within a community setting, focusing on frailty management.
- Formulate and implement treatment plans for patients on the Re-ablement ward.
- Conduct outreach visits to patients upon discharge to support rehabilitation and continuity of care.
Requirements:
- Experience in adult physiotherapy, preferably with rotations in medical, neurological, and community settings.
- Ability to develop comprehensive treatment plans tailored to individual patient needs.
- Strong communication and interpersonal skills, essential for effective patient interaction and team collaboration.
- Full registration with the Health and Care Professions Council (HCPC) as a physiotherapist.
Desirable Skills:
- Experience in managing frailty conditions.
- Knowledge of community-based rehabilitation practices.
- Ability to work independently and as part of a multidisciplinary team.
