Key Responsibilities:

  • Provide physiotherapy services within a community setting, focusing on frailty management.
  • Formulate and implement treatment plans for patients on the Re-ablement ward.
  • Conduct outreach visits to patients upon discharge to support rehabilitation and continuity of care.

Requirements:

  • Experience in adult physiotherapy, preferably with rotations in medical, neurological, and community settings.
  • Ability to develop comprehensive treatment plans tailored to individual patient needs.
  • Strong communication and interpersonal skills, essential for effective patient interaction and team collaboration.
  • Full registration with the Health and Care Professions Council (HCPC) as a physiotherapist.

Desirable Skills:

  • Experience in managing frailty conditions.
  • Knowledge of community-based rehabilitation practices.
  • Ability to work independently and as part of a multidisciplinary team.
