Key Responsibilities
- Conduct comprehensive assessments of patients to determine their occupational needs and goals related to their medical conditions.
- Develop and implement individualised treatment plans in collaboration with patients and multidisciplinary teams.
- Deliver therapeutic interventions to improve patients' functional abilities and promote safe discharge planning.
- Educate patients and their families about strategies to manage daily activities and support rehabilitation efforts.
- Collaborate closely with nursing staff, physiotherapists, and other healthcare professionals to ensure a coordinated approach to patient care.
- Monitor patient progress and adjust treatment plans as necessary based on clinical outcomes.
- Maintain accurate and up-to-date patient records in compliance with departmental policies and relevant regulations.
- Participate in team meetings and contribute to quality improvement initiatives within the service.
- Provide guidance and support to junior staff and therapy students as required.
Qualifications and Experience
- Degree in Occupational Therapy and current registration with HCPC.
- Experience working in an acute hospital setting, particularly within a medicines division or related area.
- Strong clinical reasoning and assessment skills, with the ability to work effectively in a fast-paced environment.
- Excellent communication, teamwork, and interpersonal skills.
- Knowledge of discharge planning processes and rehabilitation principles.
