Key Responsibilities

  • Conduct comprehensive assessments of patients to identify their occupational needs and goals.
  • Develop and implement personalised care plans in collaboration with patients and their families.
  • Provide therapeutic interventions that enable patients to engage in meaningful activities.
  • Educate patients, families, and carers on strategies to manage daily living activities and enhance independence.
  • Work closely with other healthcare professionals within the Integrated Care Team to ensure coordinated and holistic care delivery.
  • Monitor and evaluate the effectiveness of interventions and make adjustments as necessary.
  • Maintain accurate and timely documentation in accordance with departmental policies and standards.
  • Participate in team meetings, case discussions, and contribute to service development initiatives.
  • Provide supervision and support to junior staff and students as required.

Qualifications and Experience

  • Degree in Occupational Therapy and current registration with HCPC.
  • Significant experience in a clinical setting, ideally within community or integrated care.
  • Strong assessment and intervention skills, with the ability to work autonomously.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Knowledge of relevant legislation and best practices in occupational therapy.
