Key Responsibilities
- Conduct comprehensive assessments of patients to identify their occupational needs and goals.
- Develop and implement personalised care plans in collaboration with patients and their families.
- Provide therapeutic interventions that enable patients to engage in meaningful activities.
- Educate patients, families, and carers on strategies to manage daily living activities and enhance independence.
- Work closely with other healthcare professionals within the Integrated Care Team to ensure coordinated and holistic care delivery.
- Monitor and evaluate the effectiveness of interventions and make adjustments as necessary.
- Maintain accurate and timely documentation in accordance with departmental policies and standards.
- Participate in team meetings, case discussions, and contribute to service development initiatives.
- Provide supervision and support to junior staff and students as required.
Qualifications and Experience
- Degree in Occupational Therapy and current registration with HCPC.
- Significant experience in a clinical setting, ideally within community or integrated care.
- Strong assessment and intervention skills, with the ability to work autonomously.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively within a multidisciplinary team.
- Knowledge of relevant legislation and best practices in occupational therapy.
