- Provide statutory support to adults with care and support needs, ensuring high-quality and person-centred interventions.
- Undertake assessments, support planning, and safeguarding in compliance with legal frameworks and organizational policies.
- Conduct Mental Capacity Assessments (MCA) and make Best Interest decisions in line with the Mental Capacity Act (2005).
- Manage Section 42 safeguarding inquiries, including multi-agency collaboration and adult protection plans.
- Work collaboratively with health professionals, service providers, and voluntary organizations to coordinate and deliver care.
- Maintain accurate and timely records on electronic social care databases.
- Supervise and guide colleagues, supporting professional development and ensuring high standards of practice.
- Participate in reviews of care plans to ensure outcomes are met and independence is promoted.
- Engage with carers, providing them with advice and support to continue in their roles effectively.
- Degree in Social Work or a relevant profession.
- Registered with Social Work England.
- Knowledge and post-qualified experience with learning disabilities.
- Completion of the Assessed and Supported Year in Employment (ASYE).
- Comprehensive understanding of strengths-based social work and safeguarding practices.
