Community Digital & Practice Implementation Lead (Occupational Therapist)

Contract: 3 month initial contract with the view of extension

Working hours: 22.5 hours

Daily rate: 38p/h

Location: Across Somerset County

Our Local Authority Client in Somerset are seeking a Digital Transformation and Community Digital & Practice Implementation Lead to support support the development of the developing a community provider TEC offer, Optimal Handed Care (OHC).

Key Responsibilities:

  • Lead, manage and be accountable for the work and delivery of the DISC programme and Key commissioning intentions as part of improving Practice(s) - Optimal Handed Care (OHC) Programme.
  • Support the commissioners with designing and implementing a complex programme of work, focused on Prevention.

Requirements:

  • A registered HCPC professional with a degree qualification and/or demonstrable equivalent experience including working on community and organisational transformation.
  • An interest in developing skills and practice within the care market, either within Digital/TEC and Moving & Handling.
  • Prince 2 Practitioner, Agile, Change Management or equivalent qualification or demonstrable equivalent experience.
  • Experience of leadership within Social Care and/or Healthcare, including expertise and understanding of performance and change management, business planning, budget setting and management processes.
