Band 6 - Community Mental Health Nurse/Practitioner

Search Healthcare are recruiting an experienced Community Mental Health Nurse/Practitioner(Band 6) on behalf of our client on an initial, 3 month contract with the potential of ongoing work.

Position Overview:

  • Prioritising and managing caseloads collaboratively with health professionals, utilising strong communication skills and a non-judgemental approach.
  • Conducting mental health and risk assessments to inform medical decisions, completing Care Act Assessments, and facilitating home visits for holistic support.
  • Referring patients back to primary care or other health professionals as needed, while utilising IT and multidisciplinary team channels for efficient patient screening and coordination.
  • Providing personalised support to individuals, families, and carers, empowering them to manage their health independently and achieve personalised care plan objectives.
  • Collaborating with existing services to connect patients to community activities that promote health and wellbeing autonomy.

Requirements:

-A full UK Driving Licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010

-The right to work in the UK

-Minimum 6 months experience in a UK based healthcare setting.

-current registration with the NMC and adheres to the NMC Code.

-Professional qualification in a relevant field

-Experience in mental health settings, addressing complex community issues with service users.
