Job Overview:We are seeking a skilled and motivated Band 6 Occupational Therapist to join our community team in Telford. This role involves working with patients requiring neuro rehabilitation and support for complex physical and psychological conditions, ensuring they receive comprehensive care tailored to their needs.

Key Responsibilities:

  • Conduct thorough assessments of patients with complex physical and psychological diagnoses.
  • Utilize clinical reasoning to develop and implement individualized rehabilitation and treatment plans.
  • Provide neuro rehabilitation services to support recovery and enhance functional independence.
  • Create and manage effective discharge plans in collaboration with patients, families, and the multidisciplinary team.
  • Deliver holistic community-based care, ensuring continuity and quality.

Key Requirements:

  • HCPC-registered Occupational Therapist.
  • Proven experience in community settings, including neuro rehabilitation and holistic care.
  • Strong clinical reasoning and decision-making skills.
  • Ability to manage a diverse caseload independently in a community environment.
  • Excellent communication and teamwork skills to collaborate with multidisciplinary teams.
