Job Description (short)

路 To provide a key role in facilitating the seamless transition from community hospital to patient鈥檚 own home and transferring the care of the patient to other appropriate services to maximise the patient's independence and quality of life.

路 To complete Occupational Therapy assessments of patients with diverse presentations, with complex physical and psychological diagnoses, to inform treatment planning based around agreed and realistic goals.

路 To use appropriate outcome measures in order to provide evidence of effectiveness of Occupational Therapy input.

路 To provide appropriate and timely feedback to the wider MDT to inform holistic patient care

路 To provide a high standard of Occupational Therapy care and work as an autonomous practitioner, taking responsibility for the management of patients, using robust clinical reasoning to inform care

路 To hand over care delivery as appropriate to rehabilitation assistants, ward staff and carers. To review this with an agreed timescale.

路 To work flexibly, adapting to the pressures of fluctuating clinical demand, demonstrating excellent time management and prioritisation skills

路 To develop appropriate discharge plans involving other members of the multidisciplinary team as required.
