Fantastic Clinical Pharmacist role in Telford - Primary Care.We are looking for a dynamic and enthusiastic Primary Care Pharmacist to join our multidisciplinary team in Telford. In this role, you will be at the forefront of improving patient care within primary care settings. You will collaborate closely with GP practices, healthcare professionals, and patients to provide expert pharmaceutical advice and enhance the management of long-term conditions, chronic disease prevention, and medication optimization. This is an excellent opportunity for an ambitious pharmacist who is passionate about making a tangible difference to the health and wellbeing of the local population.Key Duties
  • Medication Reviews: Conduct comprehensive medication reviews for patients with long-term conditions, optimizing their treatment plans and improving medication adherence.
  • Chronic Disease Management: Work with GP practices to manage chronic conditions such as diabetes, asthma, hypertension, and cardiovascular disease, ensuring the safe and effective use of medicines.
  • Patient Consultation: Provide expert advice to patients and healthcare professionals regarding the use of medications, side effects, and potential interactions.
  • Clinical Advice: Offer evidence-based clinical advice to GPs, nurses, and other healthcare professionals, helping to ensure the best treatment outcomes for patients.
  • Health Promotion: Play an active role in promoting preventative care and healthy lifestyle choices, including smoking cessation, weight management, and vaccination programs.
  • Medicine Optimisation: Review and manage prescribing patterns within the practice to ensure that medicines are used safely, effectively, and efficiently.
  • Multi-Disciplinary Collaboration: Work closely with a range of healthcare professionals to ensure a holistic and patient-centred approach to care.
  • Audit and Compliance: Undertake audits of medication use and prescribing patterns to support the improvement of clinical practices and ensure adherence to local and national guidelines.
  • Support and Education: Provide education and training to staff on medication-related topics and support the development of clinical pathways and guidelines.

Key Skills and Qualifications
  • Registered Pharmacist with the General Pharmaceutical Council (GPhC)
  • Experience working in a primary care or clinical setting is preferred, but not essential
  • Strong clinical knowledge and the ability to apply this in a primary care environment
  • Excellent communication and interpersonal skills with the ability to work collaboratively
  • Ability to provide tailored, patient-centred care
  • Self-motivated with strong problem-solving abilities
