Healthcare Case Manager

About the Role:

Join a dynamic team dedicated to guiding members through their healthcare journeys with expert support. As a Healthcare Case Manager, you'll assess health risks, coordinate care, and ensure members receive the best treatment within their plan’s guidelines. Your role is key in delivering a seamless, supportive, and efficient claims experience.

What You’ll Do:

  • Assess medical conditions, identifying risks and treatment options
  • Guide members through care pathways, using evidence-based guidelines
  • Liaise with private and NHS providers to arrange appropriate referrals
  • Ensure compliance with healthcare policies and maintain accurate records
  • Work closely with the onsite Functional Health Team for a 360° approach
  • Maintain cost efficiency while ensuring quality care

What You Bring:

  • Strong communication skills—empathetic, confident, and investigative
  • Knowledge of healthcare pathways, occupational health, and risk management
  • Ability to manage cases independently while knowing when to escalate
  • Proficiency in Microsoft Office and CRM systems
  • A background in healthcare, occupational health, HR, or exercise science

Qualifications:

�? Degree or equivalent experience in healthcare, exercise science, HR, or related fields�? Professional qualification in physiotherapy, nursing, CBT, nutrition, or similar is a plus
