This role involves managing complex discharge planning for individuals across multiple hospital sites within Rhondda Cynon Taff.As a member of the Hospital Discharge Team, you will play a critical role in ensuring safe, timely, and person-centred discharges from hospital settings. You will work collaboratively with healthcare professionals, families, and external agencies to support individuals transitioning from hospital to appropriate care settings, promoting their independence and well-being wherever possible.Responsibilities:
  • Conduct on-site assessments to determine the needs of patients preparing for discharge.
  • Develop and implement care plans in line with Social Care Wales standards.
  • Facilitate multi-agency collaboration to ensure safe and efficient discharge planning.
  • Work closely with health professionals to minimize delays and maximize patient outcomes.
  • Advocate for patients’ best interests, ensuring their rights and preferences are respected.
  • Address complex care needs, including support for individuals with multiple health and social care requirements.
Requirements:
  • Registered with Social Care Wales (current registration required).
  • Proven experience working within hospital discharge or complex care planning.
  • Strong understanding of relevant legislation, including the Social Services and Well-being (Wales) Act 2014.
  • Knowledge of local health and social care resources within Rhondda Cynon Taff.
  • Proficiency in IT systems used for case management and reporting.
