Social Worker, £33ph, Locality Adult Team, Vale of GlamorganPertemps are recruiting for experienced Social Workers to join the Locality Adults Services Team in Vale of Glamorgan. This is an opportunity to make a meaningful impact by supporting adults with care and support needs to achieve their personal outcomes, promoting independence, and ensuring the best possible quality of life.Key Responsibilities:
  • To undertake assessment of citizens in accordance with statutory duties under relevant legislation.
  • To plan and commission services to enable adults with care & support needs and carers with support needs to overcome the barriers to achieving the personal outcomes and monitor and review the care plans through co-production with citizens and carers
  • To work in partnership with citizens with care and support needs, carers with support needs and adults at risk to provide the best opportunities for people to live fulfilled lives in a way that matters to them.
  • To negotiate and co-ordinate applications for resources and to work in partnership with internal and external partners both statutory and voluntary on behalf of citizens and carers
  • To undertake case management responsibility for a specific caseload and prioritise workload demands appropriately.
  • To maintain up to date electronic - case files and relevant documents, providing accurate records relating to performance management and other statistical data as required.
  • To undertake training to support continuing professional development to ensure that the Council is able to retain the provision of its statutory functions, including a willingness for staff to undertake further training to become Approved Mental Health Practitioners, or other more specific, senior roles.
  • To take part in office duty rotas and other activities associated with a multi-disciplinary team.
Role requirements:
  • Social Work degree or equivalent, with registration to Social Care Wales.
  • Proven ability to assess, plan, and coordinate care and support for adults in a statutory setting.
  • Strong written and verbal communication skills, including the ability to maintain accurate records.
  • Knowledge of relevant legislation and a commitment to promoting independence and wellbeing for adults with care and support needs.
