Salary: 27,000 - 28,000 per annum

We are currently seeking a highly skilled and compassionate Care Coordinator to join our domiciliary care service based in Tunbridge Wells. As the Care Coordinator, you will play a crucial role in ensuring the smooth and efficient delivery of care services to our clients.

Key Responsibilities:

  • Coordinate and manage the delivery of care services to clients in the community
  • Create and maintain care schedules for clients, ensuring all shifts are covered
  • Liaise with clients, their families, and healthcare professionals to assess and develop care plans
  • Conduct regular reviews of care plans and make necessary adjustments
  • Monitor and maintain accurate records of client care and staff schedules
  • Provide support and guidance to care staff, ensuring they are trained and equipped to deliver high-quality care
  • Conduct regular quality assurance checks to ensure compliance with regulations and standards
  • Participate in recruitment and training of new care staff
  • Handle any client or staff concerns or complaints in a timely and professional manner
  • Collaborate with other healthcare professionals to ensure the best possible care for clients
  • Requirements:

  • Previous experience in a similar role, preferably in domiciliary care
  • Strong understanding of care regulations and standards
  • Excellent communication and interpersonal skills
