We are looking for a Youth Outreach Support Worker to join a well-established Charity in South Wales on a permanent contracted. This role is offering great package and hybrid working.

Salary: £23,982
Location: Monmouthshire
Contract: Permanent

Youth Outreach Support Worker responsibilities:

Support people aged 16-24 across Monmouthshire who are affected by homelessness and struggling to find, secure or maintain appropriate accommodation
Undertake assessments of need in accordance with the support assessment procedure
Ensure that all appropriate actions are taken to prevent unnecessary tenancy/accommodation breakdown

Youth Outreach Support Worker requirements:

At least one year experience working with individuals experience homelessness
Social housing experience
Full UK driving license
