We are seeking a dedicated Transition Coordinator to join our client's Learning Support team, working directly with young people with Special Educational Needs and Disabilities (SEND), as well as their parents/carers. This role is essential in facilitating a smooth transition to college life for students starting from Year 9 upwards.

Day-to-day of the role:
  • Collaborate with schools, specialist providers, and local authorities to arrange interviews, visits, and taster sessions for prospective students.
  • Provide advice to learners and parents on the curriculum, travel arrangements, and address specific needs during their transition to and progression from the College.
  • Administer the formal review process for learners with Education, Health and Care Plans.
  • Ensure effective communication and interpersonal interactions to build relationships and meet the diverse needs of students, parents, and stakeholders.
  • Utilise IT systems for record-keeping and administrative tasks, maintaining high levels of organisation and efficiency.
Required Skills & Qualifications:
  • Proven experience working with children or young people with SEND in an educational environment.
  • Excellent communication and interpersonal skills to engage with a diverse range of individuals.
  • Strong IT, organisational, and administrative skills.
  • Ability to prioritise and manage a busy workload effectively.
