Job Description:

Monday to Friday - 9.00am to 5.00pm - hybrid working with a minimum of 2 days home working.

To provide a Brokerage Service across Children's and Young People and Adult Social Care and Health.

To ensure the effective purchasing of care arrangements as detailed in agreed authorised care and support plans from needs led assessment, this will include the brokering of Older People and Children's Residential Homes, Children's Foster Homes, Home Care and Supported Living.

To maintain detailed records and systems for monitoring vacancies, allocations and placements across Social Care.

To negotiate fees in conjunction with advice and support.

To support with timely discharges from hospital.
