Community Care Worker **

We are seeking experienced Community Care Worker to support the Adults/Disability Services located in the Uxbridge area.

The vacancy is working on a full time agency contract for 3-6 months with a view to extended.

Rates of pay are starting at 22+ per hour which is negotiable depending on your experience.

Location is Uxbridge, UB8.

Full time position - 35 hours per week.

Main Purpose:

  • You will assist in conducting statutory Care Act Assessments for Older People and clients with Physical Disabilities, aiming to meet the 28-day performance targets and reduce waiting times for new referrals.
  • They will manage long-term, complex cases, which may involve hoarding, non-engagers, DoLS challenges, and court of protection work (e.g., welfare disputes, court applications).

Responsibilities include:

  • Completing six-week reviews.
  • Conducting Mental Capacity Assessments.
  • Performing Continuing Healthcare assessments.
  • You will be an experienced Community Care Worker with knowledge of working with Older Adults and/or People with Disabilities.
  • You will have enhanced your skills and knowledge through further training and possess specific experience with the relevant client group.
  • You will be innovative and creative, willing to explore new working models to improve outcomes for service users. Additionally, you will be a highly motivated individual capable of working as part of a team, building strong relationships to deliver the best possible outcomes for vulnerable people.
