Job Overview:We are seeking an experienced Band 7 Advanced Nurse Practitioner (ANP) to join our dynamic team at Oak Place, Halton. This role involves delivering expert care to patients in a community setting, focusing on assessment, diagnosis, and treatment across a wide range of acute and chronic conditions.

Key Responsibilities:

  • Conduct comprehensive assessments and manage patient care plans.
  • Prescribe medications and treatments within your scope of practice.
  • Collaborate with multidisciplinary teams to provide holistic care.
  • Lead and mentor junior staff, promoting clinical excellence.
  • Engage in quality improvement initiatives and evidence-based practice.

Requirements:

  • Registered Nurse with a valid NMC PIN.
