About the role:

Platinum Care at Home are seeking an organised and compassionate Care Coordinator to join our branch team in Waterlooville currently delivering in excess of 700 hours per week. In this role, you will be responsible for overseeing and coordinating the delivery of care services to our clients, ensuring they receive the highest standard of care. The ideal candidate will have excellent communication skills, a strong attention to detail, and a passion for improving the lives of others.

Key Responsibilities:

  • Coordinate and schedule care services for clients, ensuring all care plans are followed accurately.
  • Liaise with clients, families, and healthcare professionals to assess needs and develop personalised care plans.
  • Monitor and review the quality of care provided, addressing any issues or concerns promptly.
  • Maintain accurate and up-to-date client records and documentation.
  • Support and supervise care staff, providing guidance and ensuring compliance with company policies and procedures.
  • Conduct regular home visits to assess client satisfaction and well-being.
  • Participate in the recruitment and training of new care staff.

Qualifications:

  • Previous experience in a care coordination or similar role within the healthcare sector.
  • Strong organisational and time-management skills.
  • Excellent communication and interpersonal abilities.
  • Compassionate and empathetic approach to client care.
  • Ability to work effectively both independently and as part of a team.
  • Basic computer skills for documentation and scheduling purposes.
  • Relevant certification or qualification in healthcare or social care is preferred.
