Job Overview:We are seeking an experienced Band 6 Occupational Therapist to join the Acute NHS Rotational team at West Hertfordshire NHS Trust. This role involves working across wards, independently managing complex discharges, and conducting cognitive assessments to support patient care and recovery.
Key Responsibilities:
- Plan and execute complex patient discharges independently.
- Conduct cognitive assessments to inform and support treatment plans.
- Deliver therapy and interventions as part of a rotational acute ward team.
- Collaborate with multidisciplinary teams to ensure effective patient flow and quality care.
Requirements:
- HCPC registration as an Occupational Therapist.
- Minimum 6 months NHS experience, with acute rotational ward experience essential.
- Proven ability to manage complex discharge cases.
- Strong skills in cognitive assessments and care planning.
