Key Responsibilities:We are seeking a highly skilled and motivated Band 8a Pharmacist to join our dynamic team at Watford General Hospital. The successful candidate will play a crucial role in delivering high-quality pharmaceutical care to our patients and supporting our clinical teams. Responsibilities include:
- Medicines Reconciliation: Ensuring accurate and timely reconciliation of medicines for patients during admission and discharge.
- Patient Counselling: Providing expert advice to patients regarding their medications to ensure safe and effective use.
- Advice to Healthcare Professionals: Offering professional advice and guidance to doctors and nurses on the safe and effective use of medicines.
- Screening Drug Charts and TTAs: Reviewing drug charts and "to-take-away" (TTA) prescriptions to ensure compliance with clinical guidelines and patient safety.
- Clinical Interventions: Identifying and implementing interventions to optimize therapeutic outcomes and minimize risks associated with drug therapy.
Requirements:
- Registration with the General Pharmaceutical Council (GPhC) as a practicing pharmacist.
- Significant experience in hospital pharmacy, ideally at Band 8a level or equivalent.
- Strong knowledge of medicines management, clinical guidelines, and pharmaceutical care processes.
- Excellent communication skills, with the ability to work collaboratively within a multidisciplinary team.
- A proactive approach to problem-solving and a commitment to continuous professional development.
