We are looking for a Band 6/7 Clinical Pharmacist to join our team across four community hospitals in North Powys: Welshpool, Newtown, Machynlleth, and Llanidloes. This is an exciting opportunity to provide clinical ward cover for a variety of patient groups, including Care of the Elderly, Medical, Palliative Care, and Rehabilitation patients.

As part of this role, you will work alongside a skilled team of pharmacy technicians and assistants, as well as the wider multidisciplinary team (MDT), including GPs and therapy teams. You will have support from a dedicated team of pharmacists and other pharmacy professionals.

Key Responsibilities:

  • Provide clinical pharmacy services across the four hospital sites.
  • Work collaboratively with the MDT to deliver high-quality patient care.
  • Review and manage medication therapy for elderly, medical, palliative, and rehabilitation patients.
  • Contribute to the development and delivery of clinical guidelines and protocols.
  • Offer expert advice on medication-related issues to healthcare professionals.
  • Participate in multidisciplinary ward rounds and patient care discussions.

Essential Qualifications/Experience:

  • Registered Pharmacist with the General Pharmaceutical Council (GPhC).
  • Experience in clinical pharmacy, ideally in a hospital or community setting.
  • Excellent communication skills, both written and verbal.
  • Ability to work independently and as part of a team.
  • Flexibility and willingness to travel between hospital sites as required.
