Job description:
- This is a dynamic post that requires the post holder to work closely with uni/multi-disciplinary teams/agencies in addressing a range of care/support issues preventing unnecessary hospital admissions and improving the health and wellbeing of the population served.
- The Team supports adults within their own homes where the primary need is physical disability with long-term health conditions.
- The aim of OT intervention is to assist people to remain as independent as possible in the community.
- Applicants would benefit from having worked in a social care Occupational Therapy role previously or have experience of the community setting.
