Occupational Therapist Band 6

Meridian Business Support is currently recruiting for a qualified band 6 Occupational Therapist experience preferred in Intermediate care, rapid response and hospital discharge. Will be based in an Intermediate care service and working within patients own homes and in Intermediate care bed based units, they will complete therapy assessments, carry out rehabilitation interventions, provide crisis interventions and attend multidisciplinary meetings to improve outcome for service users and reduce length of stay in this service area To have a hands on approach with rehabilitation goals and treatments and be able to complete and update care plans for rehabilitation care staff to follow. Communicate regularly and effectively in order to maximise patient rehab potential and complete paperwork efficiently and comprehensively. Be able to work both independently and as an effective team member alongside other professionals within health and social care services.The role is for a Temporary worker and would be expected to work 37.5 hours per week Monday to Friday
