Role and Responsibilities:
  • Deliver high-standard assessments and rehabilitation interventions in an intermediate care setting.
  • Develop and implement individualized treatment plans, focusing on maximizing patient independence and safety.
  • Work collaboratively with a multidisciplinary team, including nurses, social workers, and other allied health professionals, to provide comprehensive care.
  • Manage a diverse caseload, ensuring timely and effective care delivery for community-based and inpatient services.
  • Educate and support patients and their families in understanding treatment plans and rehabilitation goals.
  • Contribute to the development and review of clinical protocols and best practices.
Requirements:
  • Diploma/Degree in Occupational Therapy or Physiotherapy.
  • HCPC Registration.
  • Experience in intermediate care or rehabilitation settings.
  • Ability to effectively manage care for frail and elderly patients.
  • Strong community experience and a proactive approach to patient care.
  • Excellent communication and interpersonal skills.
Skills:
  • Strong clinical skills in assessing and treating a range of conditions.
  • Ability to detect patient deterioration.
  • Ability to risk assess patient assessment, treatment, and discharge.
  • Proficiency in assessing and treating motor, sensory, and physical functioning.
  • Ability to assess and treat cognitive and affective impairments and their impact on patient function.
  • Clear clinical reasoning in structuring holistic assessments and OT processes.
  • Evidence of Continued Professional Development (CPD).
