Role and Responsibilities:- Deliver high-standard assessments and rehabilitation interventions in an intermediate care setting.
- Develop and implement individualized treatment plans, focusing on maximizing patient independence and safety.
- Work collaboratively with a multidisciplinary team, including nurses, social workers, and other allied health professionals, to provide comprehensive care.
- Manage a diverse caseload, ensuring timely and effective care delivery for community-based and inpatient services.
- Educate and support patients and their families in understanding treatment plans and rehabilitation goals.
- Contribute to the development and review of clinical protocols and best practices.
- Diploma/Degree in Occupational Therapy or Physiotherapy.
- HCPC Registration.
- Experience in intermediate care or rehabilitation settings.
- Ability to effectively manage care for frail and elderly patients.
- Strong community experience and a proactive approach to patient care.
- Excellent communication and interpersonal skills.
- Strong clinical skills in assessing and treating a range of conditions.
- Ability to detect patient deterioration.
- Ability to risk assess patient assessment, treatment, and discharge.
- Proficiency in assessing and treating motor, sensory, and physical functioning.
- Ability to assess and treat cognitive and affective impairments and their impact on patient function.
- Clear clinical reasoning in structuring holistic assessments and OT processes.
- Evidence of Continued Professional Development (CPD).
