Job Overview:We are seeking a dedicated and experienced Senior Occupational Therapist to join the team at Wrightington Wigan and Leigh NHS Trust. This role provides a unique opportunity to work in a dynamic setting dedicated to delivering high-quality care and rehabilitation services to frail and elderly patients.

Role and Responsibilities:

  • Deliver high-standard assessments and rehabilitation interventions to patients in an intermediate care setting.
  • Develop and implement individualized treatment plans, focusing on maximizing patient independence and safety.
  • Work collaboratively with a multidisciplinary team, including nurses, social workers, and other allied health professionals, to provide comprehensive care.
  • Manage a diverse caseload, ensuring timely and effective care delivery for community-based and inpatient services.
  • Educate and support patients and their families in understanding treatment plans and rehabilitation goals.
  • Actively contribute to the development and review of clinical protocols and best practices.

Requirements:

  • Diploma/Degree in Occupational Therapy or Physiotherapy.
  • HCPC Registration.
  • Experience in intermediate care or rehabilitation settings.
  • Ability to effectively manage care for frail and elderly patients.
  • Strong community experience and a proactive approach to patient care.
  • Excellent communication and interpersonal skills.

Skills:

  • Strong clinical skills in assessing and treating a range of conditions.
  • Ability to detect the deteriorating patient.
  • Ability to risk assess patient assessment, treatment, and discharge.
  • Ability to assess and treat motor and sensory skills as well as physical functioning.
  • Ability to assess and treat cognitive and affective impairment and its impact on function and the patient's ability to maintain their health and safety.
  • Ability to structure holistic assessment and the OT process within a framework that provides clear clinical reasoning for recommendations made in relation to a patient's needs and care pathway.
  • Evidence of CPD.
