Intermediate Care - Bed Based & Home Based Therapy

• Diploma / Degree in Occupational therapy.

• HCPC Registration• UK NHS Experience• Post registration experience in rehabilitation and condition management.

The Occupational Therapist in Intermediate Care provides both bed-based and home-based therapeutic services to adult patients transitioning from hospital to home. This role involves assessing, planning, and implementing therapy plans that support patients in regaining or maintaining their functional independence. Working as part of a multidisciplinary team, the therapist will focus on optimizing patients' physical, mental, and social abilities, allowing them to live safely and independently within their community.

Key Responsibilities:
  • Assessment and Planning: Conduct comprehensive assessments of patients' physical, cognitive, and functional abilities within bed-based and home-based settings.

    • Evaluate patients’ needs for adaptive equipment, mobility aids, and environmental modifications.
    • Develop individualized treatment plans focused on improving daily living skills, mobility, and safety.
