Job Overview

This role involves providing physiotherapy to elderly patients within the Jean Hayes Reablement Unit. The focus is on rehabilitation and community care to help patients regain independence and enhance their quality of life.

Key Responsibilities
  • Conduct assessments and deliver physiotherapy treatments tailored to elderly patients.
  • Develop individualized rehabilitation plans in collaboration with the multidisciplinary team.
  • Undertake community visits and provide home-based physiotherapy as required.
  • Monitor and evaluate patient progress, adjusting treatment plans as needed.
  • Facilitate safe discharge planning and provide guidance to families and caregivers.

Requirements
  • HCPC-registered Physiotherapist.
  • Proven experience in elderly care, rehabilitation, and community physiotherapy.
