This is a fast paced role within adult Social Care working under the Adult Front Door model of practice, working within an asset based approach.

Duties of the CAF (Community Adult Front Door) Social Worker:

Conducting assessments to identify risks for vulnerable adults.

Developing and implementing care plans tailored to individual needs, in collaboration with residents and their support networks.

Conducting home visits and telephone assessments and contact with residents to monitor their safety and well-being.

Coordinating with external agencies, healthcare professionals, and community resources to provide holistic support to residents.

Participating in multidisciplinary meetings to review resident progress and develop intervention strategies.

Providing crisis intervention and support to residents experiencing immediate safety concerns.

Maintaining accurate and up-to-date records of resident interactions, assessments, and interventions in accordance with regulatory standards.
