- Conducting assessments to identify risks for vulnerable adults.
- Developing and implementing care plans tailored to individual needs, in collaboration with residents and their support networks.
- Conducting home visits and telephone assessments and contact with residents to monitor their safety and well-being.
- Coordinating with external agencies, healthcare professionals, and community resources to provide holistic support to residents.
- Participating in multidisciplinary meetings to review resident progress and develop intervention strategies.
- Providing crisis intervention and support to residents experiencing immediate safety concerns.
- Maintaining accurate and up-to-date records of resident interactions, assessments, and interventions in accordance with regulatory standards.
- Degree or equivalent in social work
- Current Social Work England registration
- Significant adult social care and crisis intervention experience (minimum 3 years)
