This is a brand-new role to the Trust where you will be able to make your mark, putting your own stamp on the role

Job SummaryAs a Care Coordinator, you will be responsible for overseeing and coordinating care services for individuals in need of assistance. You will play a crucial role in ensuring that clients receive the appropriate support and services to enhance their well-being.

Duties- Assessing clients' care needs and developing personalised care plans- Coordinating with healthcare professionals, social workers, and family members to provide comprehensive care- Monitoring and evaluating the effectiveness of care plans- Scheduling appointments and arranging transportation for clients- Maintaining accurate records of client interactions and progress- Providing emotional support and guidance to clients and their families

You'll learn the following whilst working at the trust:

1. An in depth understanding of the roles and responsibilities involved in working within the NHS

2. Knowledge of the systems used, to effectively always complete your role to the highest standard

3. A sense of teamwork, gained through working alongside and supporting colleagues from all levels, within the organisation

SkillsEducated to NVQ 3 Level or equivalent i n a relevant subject i.e. Administration, IT or Health/ or Social Care or equivalent level of qualification or significant equivalent previous proven experience.

Use to working in a busy, challenging environment

Intermediate knowledge of IT systems, databases and software pro-grams such as Outlook, Word, Excel, and Power-point. NHS systems desirable
