Occupational Therapist- Band 6

Location: Woking Community Hospital

Working Hours : 37.5hours

Start date: ASAP End date: Ongoing

Position Summary:

We are seeking a compassionate, skilled Community Occupational Therapist to join our team. The Community OT will provide personalized, community-based occupational therapy services aimed at promoting independence, enhancing quality of life, and supporting individuals with physical, cognitive, or emotional challenges. This position involves working with clients across a variety of community settings, including homes, schools, workplaces, and community centers. The ideal candidate will excel at developing individualized treatment plans, advocating for client needs, and working collaboratively with multidisciplinary teams.

  • Key Responsibilities:
    1. Client Assessment and Goal Setting

      • Conduct comprehensive assessments of clients' physical, cognitive, and emotional needs within their community environments.
      • Develop and document individualized care plans and set achievable, client-centered goals in collaboration with clients and their families
