Job Description:

Holden Knight Healthcare is excited to represent a Local Council seeking to hire a Senior Social Care Practitioner for their Transition Team. This role offers a fantastic opportunity to join a dynamic team dedicated to supporting young people transitioning from children’s to adult services.

As a Senior Social Care Practitioner, you will play a pivotal role in delivering high-quality care assessments and person-centred plans. You will work closely with young people and their families, helping them navigate the transition process smoothly while ensuring that all their care needs are met.

Key Responsibilities:

  • Conduct assessments of children and young people transitioning into adult care services.
  • Develop and implement person-centred care plans that address individual needs.
