Wokingham Borough Council are recruiting for an adult Social Worker to join their early support team. This is a full-time locum role, working 37 hours a week.
The Role:
To work within the relevant legal frameworks, guidance, policies, and procedures to undertake assessments, reviews and support planning with individuals and their carers, promoting well-being, independence and safety.
To enable people to identify their own needs and the outcomes they wish to achieve and to implement and review support plans to meet these needs and to liaise with other professional/agencies as required.
Essential Requirements:
* Social Work Qualification
* Experience as a social worker within adult social care teams
* Social Work England Registration
* Driving license and access to own vehicle
