Placement Officer

Wokingham Borough Council are looking for Placement Officer to join their Adult Social Care Team. As a Placement Officer you will:

  • Be responsible for sourcing placements for vulnerable adults, including those ready to be discharged from the hospital
  • Work closely with Social Workers and care providers to negotiate fees and secure appropriate placements, all while ensuring the best outcomes for the people supported
  • Provide duty cover, triaging and redirecting emails and responding to urgent queries

Requirements

  • Experience in a social care setting, understanding the complexities of adult care placements
  • Experience in sourcing placements for vulnerable adults
  • Confident in liaising with Social Workers and care providers via telephone and other channels
  • Able to handle challenging situations and navigate complex care requirements
