- Undertake assessments for referred cases, collaborating with care workers and clinicians to ensure comprehensive evaluations.
- Make informed recommendations regarding eligibility for health funding (e.g., CHC, S117, Continuing Care) and proposed funding allocations.
- Support the Team Manager in addressing operational issues, ensuring high levels of team and individual performance.
- Assist with team coordination and resource planning for day-to-day operations.
- Collaborate with the Team Manager to design and deliver training programs for council staff and, where applicable, health partners.
- Focus on improving understanding and application of CHC, S117, Continuing Care, and children’s funding streams.
- Degree in Social Work with active registration to practice.
- Significant experience in conducting complex social care assessments and working with multi-disciplinary teams.
- Strong understanding of CHC, S117, and Continuing Care frameworks.
