Senior Social Worker, £38ph, CHC Team, WokinghamPertemps are recruiting for an experienced Senior Social Worker to join the All Age CHC team in Wokingham. This role offers an opportunity to work on complex assessments and cases while supporting operational excellence and contributing to training initiatives within the council.Job Purpose:To investigate referrals, conduct complex assessments, and make recommendations regarding eligibility for health funding and proposed funding splits under agreed frameworks and criteria.Key Responsibilities:
  • Undertake assessments for referred cases, collaborating with care workers and clinicians to ensure comprehensive evaluations.
  • Make informed recommendations regarding eligibility for health funding (e.g., CHC, S117, Continuing Care) and proposed funding allocations.
  • Support the Team Manager in addressing operational issues, ensuring high levels of team and individual performance.
  • Assist with team coordination and resource planning for day-to-day operations.
  • Collaborate with the Team Manager to design and deliver training programs for council staff and, where applicable, health partners.
  • Focus on improving understanding and application of CHC, S117, Continuing Care, and children’s funding streams.
Role requirements:
  • Degree in Social Work with active registration to practice.
  • Significant experience in conducting complex social care assessments and working with multi-disciplinary teams.
  • Strong understanding of CHC, S117, and Continuing Care frameworks.
