Social Prescribing Driver Link Worker | Based at WVCA, Wolverhampton with city-wide travel | 37 hours a week | The post is offered on a fixed-term basis until 31st March 2025 | £24,964 per annum, pro-rata (Plus Driving Allowance up to £2,500 p.a. pro rata; mileage paid at 45p per mile)
This role aims to strengthen collaboration between health, social care, public health, and the Voluntary and Community sector to support patients with non-medical needs. Social Prescribing allows GPs to refer patients to local, non-clinical services, delivered by Link Workers, who provide holistic support to help clients manage their health and connect with community resources.

Requirements
- Clean driving licence, daily vehicle access, and business insurance.
- NVQ Level 3, Degree, or equivalent experience.
- Proven experience in client-centered roles, community development, or health/social care.
Skills
- Strong communication and networking, ability to manage risks, and experience working with diverse communities.

Your role will include but not be limited to
Client Engagement
- Take referrals and provide transportation to appointments or community activities.
- Build trust with clients by offering one-on-one support, following a care plan tailored to each person’s needs.
- Conduct meetings in locations that best suit clients, including GP surgeries and homes.
- Collaboration with Link Workers based at WVCA
Team Coordination
- Provide updates to Link Workers and collaborate with GPs, social care professionals, and VCO providers.
Other Duties
- Manage client appointment schedules, attend team meetings, and document activities.
- Ensure timely case studies, reports, and contribute to WVCA’s evolving Social Prescribing Service.
