- Undertake Care Act assessments and Mental Capacity Act conversations to identify and address the needs of young people transitioning to adult services.
- Work collaboratively with young people, families, and professionals to ensure person-centred plans that maximise independence and well-being.
- Manage a caseload of young people with varying disabilities and health conditions, providing tailored support and advice.
- Apply relevant legislation, including the Deprivation of Liberty Safeguards (DoLS), to ensure compliance and safeguard individual rights.
- Maintain accurate case records and provide timely reports in line with statutory requirements.
- Participate in monthly supervision to reflect on and enhance practice.
- Qualified and registered with Social Work England.
- Experience working within Adult Social Care, particularly in transitions or disabilities services.
- Experience of DoLS preferable
- Strong understanding of the Care Act 2014, Mental Capacity Act 2005, and DoLS.
- Excellent communication, assessment, and planning skills.
- Ability to work collaboratively and independently in an agile working environment.
