Position: Experienced Social WorkerLocation: WolverhamptonTeam: Adults Transitions Service (18–25)Rate: £30.57 per hour Working Arrangement: Agile working (office presence required 2 days per month)Role OverviewPertemps is recruiting for a dedicated and experienced Adult Social Worker to join its Adults Transitions Service, focusing on supporting young people aged 18–25 with a range of disabilities and health needs. This vital role ensures that young people transition smoothly into adult services through the application of the Care Act 2014 and Mental Capacity Act 2005.Key Responsibilities:
  • Undertake Care Act assessments and Mental Capacity Act conversations to identify and address the needs of young people transitioning to adult services.
  • Work collaboratively with young people, families, and professionals to ensure person-centred plans that maximise independence and well-being.
  • Manage a caseload of young people with varying disabilities and health conditions, providing tailored support and advice.
  • Apply relevant legislation, including the Deprivation of Liberty Safeguards (DoLS), to ensure compliance and safeguard individual rights.
  • Maintain accurate case records and provide timely reports in line with statutory requirements.
  • Participate in monthly supervision to reflect on and enhance practice.
Requirements:
  • Qualified and registered with Social Work England.
  • Experience working within Adult Social Care, particularly in transitions or disabilities services.
  • Experience of DoLS preferable
  • Strong understanding of the Care Act 2014, Mental Capacity Act 2005, and DoLS.
  • Excellent communication, assessment, and planning skills.
  • Ability to work collaboratively and independently in an agile working environment.
