  • Competitive pay rate of £17.59 per hour
  • Full-time position, working 37 hours per weekThe duties of this role include:
  • Manage a caseload of care leavers, providing tailored support based on their individual needs
  • Collaborate with social workers to co-work Eligible Children in care
  • Help develop Pathway Plans and conduct regular reviews
  • Ensure that care leavers receive the necessary support and resources for a successful transition to independence
  • Deliver group sessions, activities, and personal development programmes for young people
  • Liaise with internal and external stakeholders to promote joint work and access additional support services
  • Monitor and support care leaver's educational achievements, including those in further and higher education
  • Maintain accurate and up-to-date records and generate reports as required
  • Relevant experience in working with young people, particularly those leaving care
  • Sound knowledge of legislation and policies related to young people leaving care
  • Strong communication and interpersonal skills to engage with young people and build rapport
  • Ability to work collaboratively within a multidisciplinary team
