- Competitive pay rate of £17.59 per hour
- Full-time position, working 37 hours per weekThe duties of this role include:
- Manage a caseload of care leavers, providing tailored support based on their individual needs
- Collaborate with social workers to co-work Eligible Children in care
- Help develop Pathway Plans and conduct regular reviews
- Ensure that care leavers receive the necessary support and resources for a successful transition to independence
- Deliver group sessions, activities, and personal development programmes for young people
- Liaise with internal and external stakeholders to promote joint work and access additional support services
- Monitor and support care leaver's educational achievements, including those in further and higher education
- Maintain accurate and up-to-date records and generate reports as required
- Relevant experience in working with young people, particularly those leaving care
- Sound knowledge of legislation and policies related to young people leaving care
- Strong communication and interpersonal skills to engage with young people and build rapport
- Ability to work collaboratively within a multidisciplinary team
