What you will do:

  • Administer comprehensive assessments to determine patient needs and develop individualised care plans.
  • Provide therapeutic interventions and support to individuals with complex mental health needs.
  • Work collaboratively within the Community Mental Health Team to deliver holistic care and support.
  • Participate in multidisciplinary team meetings and contribute to the development of innovative mental health services.
  • Maintain accurate records and ensure compliance with relevant procedures and standards.
