What you will do:
- Administer comprehensive assessments to determine patient needs and develop individualised care plans.
- Provide therapeutic interventions and support to individuals with complex mental health needs.
- Work collaboratively within the Community Mental Health Team to deliver holistic care and support.
- Participate in multidisciplinary team meetings and contribute to the development of innovative mental health services.
- Maintain accurate records and ensure compliance with relevant procedures and standards.
