About the Role:Worcestershire County Council is seeking a passionate and dedicated Personal Advisor to join their Care Leavers Service. In this key role, you will support young people aged 17-24 to transition successfully to adulthood. You will work closely with young people, placing them at the heart of decision-making, and help them achieve independence and a bright future.

You will be the allocated worker and key contact for your young people, offering practical, personal, and emotional support. This is a dynamic role where no two days are the same, and you will have the opportunity to build meaningful relationships while contributing to positive outcomes.

Key Responsibilities:

  • Develop and maintain respectful, supportive relationships with young people
  • Create and implement Pathway Plans in collaboration with young people
  • Keep accurate case records and maintain up-to-date documentation
  • Complete regular visits to young people at home and in the community
  • Attend meetings and work closely with partner agencies to ensure the best possible outcomes for your young people
